You must have completed a COVID-19 vaccination series and provide documentation to avoid the risk of being denied clinical placement.
A COVID-19 vaccination series is either two doses in a 2-dose series (such as Pfizer-BioNTech or Moderna) or a single-dose vaccine (such as Janssen/Johnson & Johnson).
Some students cannot be vaccinated because of identified medical exemptions, medical deferments or religious accommodations and should be exempted or deferred from receiving the vaccine. If you believe you should be exempt from this vaccine for any of these reasons, provide a copy of the exemption/deferral policy.
Submit your American Heart Association BLS Provider CPR certification.
OR
American Heart Association authorized provider BLS certification.
Please avoid physical and online sites that use the verbiage "In accordance with current AHA guidelines." These are not AHA authorized provideres.
The AHA distributes electronic completion cards. This can be directly uploaded. If you received an actual card, make sure to sign the card and submit a copy of both side; front and back.
RENEWAL
The renewal will be set based on the expiration of your current certification.
After acceptance into your Health Program of interest, you will be given a handbook for that program. You will read the handbook, then sign the appropriate form. Prior to upload, ensure the Department Chair or Program Chair/Coordinator does not require their signature as well to meet this requirement. Once completed to the Program Chair/Coordinator’s standards, you will then upload the signed document.
The following are required:
3 vaccinations
AND
a positive antibody titer (lab report required)
If your titer is negative or equivocal, new alerts will be created for you to repeat the vaccination series (3 vaccines administered after your titer).
If your series is in process, submit where you are in the series, and new alerts will be created for you to complete the process.
Note for Students:
The Hepatitis B Vaccine should be administered according to the following schedule:
Vaccine 1: Birth or Anytime
Vaccine 2: at least 1 month after vaccine 1
Vaccine 3: at least 5 months after vaccine 2
Submit documentation of the following:
A flu vaccine administered during the current flu season (August - May).
RENEWAL
The renewal date will be set for October 30th.
One of the following is required:
2 vaccinations
(Vaccinations can be a combined MMR vaccination, however if individualized vaccinations are submitted you MUST submit 2 vaccinations for Mumps, 2 vaccinations for Measles and 2 vaccinations for Rubella.)
OR
Positive antibody titer (lab report required) for all 3 components.
If your titer is negative or equivocal, new alerts will be created for you to repeat the vaccination series (2 vaccines administered after your titer).
If your series is in process, submit where you are in the series, and new alerts will be created for you to complete the process.
Note for Students:
The MMR Vaccine should be administered according to the following schedule:
Vaccine 1: Birth or Anytime
Vaccine 2: at least 1 month after vaccine 1
Download, print, complete, and re-submit the 1 page physical exam form. Documentation must be signed by a medical professional and be dated within the past 6 months.
Physical Exam Form
You will purchase medical liability on campus from the Cashier’s Office. There are two locations- in the Tony Rand Student Center and in the Thomas McLean Administration Building. You will upload your receipt for this purchase in this section to complete this requirement.
Submit documentation of one of the following:
A Tdap vaccine administered within the past 10 years
OR
A Tdap vaccine administered within your lifetime AND Td booster(s) administered within every 10 years of initial Tdap.
If your documentation lists multiple Tetanus vaccinations on one line (e.g. Tetanus/DTP/DTaP/Td/Tdap), ensure that your physician indicates which shot was administered on each date listed.
RENEWAL
The renewal date will be set for 10 years from the most recent vaccine.
All vaccinations must be provided.
*New DHHS recommendation being adhered to by the Health Department: any recent TD immunizations will receive the TD booster. *
One of the following completed within the past 12 months is required:
2-step TB skin test (administered 1-3 weeks apart)
OR
blood test (lab report required)
OR
a clear chest x-ray (lab report required).
If positive results, submit physician clearance documented on letterhead.
Note for Students:
If you have history of positive TB results you will need a licensed medical professional to complete the N.C. Department of Health and Human Health Services form #3405, “Record of Tuberculosis Screening” and upload.
RENEWAL
TB test is valid for 12 months.
Upon renewal, one of the following is required:
1 step TB skin test
OR
blood test (lab report required)
OR
a clear chest x-ray (lab report required).
If positive results, submit physician clearance documented on letterhead.
* Some facilities require a 2-Step TB test. Please follow instructions accordingly.
One of the following is required:
2 vaccinations
